- According to explicit (f. 200) and note (front pastedown), written in Cologne in 1452 by student Theodoric of Alkmaar for the Carthusian monastery of St. Barbara in Cologne.
Eight-line red initial with purple pen flourishes, colored initials and rubrics throughout text.
